How do I open and fill out a Questionnaire as a Client?

When your practitioner sends you a questionnaire, you’ll get an email. You can either log in to the client portal or use the secure link in the email to open and fill it out — no login required if you use the secure link.

Access the Questionnaire

  1. Open the email from your practitioner or log in to the client portal.

  2. In the email, choose:

    • Fill out securely — opens in a new tab (no login needed), or

    • Log in and submit — takes you to the portal login.

  3. If you choose Log in and submit:

    • Click View questionnaire to get started.

  4. If you chose Fill out Securely:

    • Read the description and click Done to start filling it out.


Fill Out the Questionnaire

  1. When the questionnaire opens, choose:

    • Pre-fill — auto-fills available fields from your client record.

    • Fill out manually — enter all answers yourself.

  2. If you used Pre-fill, edit any field as needed by clicking the (x) next to it and manually typing in an answer.

  3. You can switch between Pre-fill and manual at any time by clicking the Add Responses or Remove Responses button at the top of the page.

    • Add Responses will display if you have not used the pre-fill option yet.

    • Remove Responses allows you to undo pre-filled responses.

  4. Use Next in the bottom-right to move through pages, or select sections from the left-side menu.

  5. Each page may offer Pre-fill if data is available; otherwise, fill it in manually.

Save or Submit

  1. Click Save (upper right) to save progress and finish later.

  2. Click Submit (upper right) to send the questionnaire to your practitioner.
